Your privacy is important to Extendicare. We’re committed to protecting the privacy of any personal information you may provide when visiting our website. Our information management practices have been designed to ensure compliance with all applicable federal and provincial privacy legislation.
The following policy will tell you what information we collect about you, how we protect it and what choices you have about how that information is used.
Information Collected
We may collect personal information from Users in a variety of ways, including, but not limited to, when Users fill out a form, and apply for a job. We will collect personal information from Users only if they voluntarily submit such information to us. We do not provide your information to any outside vendors or individuals.
We will collect non-personal information about Users whenever they interact with our Site; anonymous "usage" information is collected, such as the type of browser you use, the pages you visit, and your source domain. We use this information to monitor traffic and site activity, and to improve the performance and content of our site.
This information will never be associated with you as an individual or associated with any other information you may submit through the site. General demographics may be shared with our business partners or marketing but this information can not be personally linked to anyone.

Extendicare may collect and use Users' personal information to:

We employ cybersecurity and data security practices and processes to ensure there is no unauthorized access to your personal information. Any information that you voluntarily share with Extendicare is kept strictly confidential and fully secure. Only authorized persons are permitted to access the information you submit.

Extendicare may use web site “cookies” in the process of compiling the statistical information mentioned above and to enhance the user experience, tailoring certain pages of the site for your browsing convenience. You have the option of setting your browser to reject cookies, but our site may not function correctly if you do.
Third party websites
Our web sites may contain links to other 3rd party web sites. Extendicare is not responsible for the security or privacy practices of these sites, the products or services offered by these sites, or the content appearing at these sites. These third party sites may have their own privacy and customer service policies and procedures; viewing these sites is subject to their policies and procedures as applicable.
Resident / Patient Health Record Request and Health Record Correction Request
Every resident/patient/SDM* has the right to access their health record or request a correction to their health care record.
Health Record Access Request
At any time, the resident/ patient or SDM may request to view the health record. If the resident/patient/SDM would like a copy of the health record, they may make a formal request. If the request is coming from the SDM, the home or site will verify the legal documents to confirm SDM status prior to the release of the records.
The resident/patient/SDM will be provided with a copy of the requested health records or a detailed search response within 30 days of receiving the request. The home/district office will advise if an extension of time is required.
A nominal processing fee may be charged for providing the health record request. An estimate of fees will be provided before processing your request.
Health Record Correct Request
Similar to the health record access request, at any time the resident/patient or SDM may request to change factual information in their health record. This request must be provided in writing to a member of management in the home or site, detailing the information wishing to be corrected in the health record. No opinions, assessments or information not created by Extendicare or its staff can be corrected.
In the event that a resident/patient has deceased, the Estate Trustee must make the request for the health records or record correction. After the Estate Trustee has been verified from the resident/patients Last Will and Testament, health records may be released or the correction may be completed as required. If there is no named Estate Trustee, the person who has assumed responsibility for the administration of the deceased's estate must make the request.
*A resident or patient who is capable must request or approve the request for access to their health care record. A SDM is not active unless the resident/patient has been deemed incapable or is below the age of 16.

You may direct any inquiries about our privacy practices to our Privacy office at Privacy_Officer@Extendicare.com.
You may direct any inquiries or requests for your own personal health information record/health chart, to a manager at your home/ local district office.
If you are unable to resolve your concern by working with our Privacy Office or wish to make a complaint related to privacy and confidentiality, you may contact your Provincial privacy office. Click on your Province for the information on who to contact.
